This role will be performed remotely, offering flexibility while enabling the LCSW to engage with patients and families using telehealth platforms. Key Responsibilities\n

• Conduct thorough psychosocial assessments that identify the emotional, social, and financial needs of patients.

• Collaborate closely with medical teams—physicians, nurses, chaplains, and other specialists—to develop and implement holistic care plans.

• Provide individual and group counseling sessions that focus on adjustment, grief, and coping strategies for patients and their families.

• Coordinate with community resources and organizations to facilitate post-discharge services, ensuring continuity of care and support.

• Offer educational resources and emotional guidance regarding treatment options, coping mechanisms, and available support services.

• Engage in crisis intervention and provide timely emotional support to patients facing difficult decisions related to their care.

• Develop discharge plans that connect patients with community resources and support networks.

• Document all interventions, assessments, and patient interactions meticulously, adhering to hospital policies, legal requirements, and best practices.

Patient Population / Client Focus\n

The primary focus of this role will be on patients receiving hospice care, navigating the complex emotional landscapes associated with terminal illnesses. The LCSW will work closely with diverse patient populations, including patients from marginalized communities. Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Though two years of experience in a healthcare or hospital setting is preferred, candidates with relevant experience in hospice care, community mental health, or social services will also be considered favorably.
