About the Role:

Virtue Community Care Inc. is seeking a dependable and organized Case Manager to support youth placed through the Tennessee Department of Children’s Services (DCS) in our Supported Living and Family Model programs.

The Case Manager is responsible for coordinating services, monitoring youth progress, maintaining required documentation, and serving as a key liaison between the youth, DCS, caregivers, residential staff, schools, healthcare providers, and other members of the treatment team.

Responsibilities:

• Manage an assigned caseload of DCS youth and maintain regular contact with each youth.
• Coordinate with DCS Family Service Workers, caregivers, residential staff, schools, therapists, medical providers, and other members of the youth’s support team.
• Participate in treatment planning, Child and Family Team Meetings, discharge planning, and other required meetings.
• Monitor each youth’s behavioral, educational, medical, emotional, and independent living needs.
• Complete monthly summaries, case notes, assessments, treatment plan updates, and other required documentation accurately and on time.
• Monitor appointments, medications, school participation, incidents, family contacts, and progress toward treatment goals.
• Assist with crisis response, safety planning, placement stabilization, and follow-up after significant incidents.
• Ensure case records remain compliant with DCS requirements and agency policies.
• Advocate for the youth’s needs while promoting safety, stability, permanency, and successful transition toward adulthood.

Requirements:

• Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, Criminal Justice, or a related field preferred.
• Experience working with children or adolescents in foster care, residential services, behavioral health, child welfare, or a related setting strongly preferred.
• Strong documentation, organization, communication, and time-management skills.
• Ability to manage multiple cases and meet required deadlines.
• Ability to work collaboratively with DCS and multidisciplinary treatment teams.
• Valid driver’s license, reliable transportation, and ability to meet agency background and employment requirements.
