Community Case Navigator & Resource Connector
CAMBA
CAMBA is seeking a Case Management Navigator to support the Van Dyke Cornerstone Community Center team in Brooklyn. You will work with a Social Worker to implement ISPs, provide outreach, and connect participants with health, housing, and employment resources. You will coordinate with Cure Violence partners, identify high-risk participants, and maintain trust to ensure meaningful engagement while supporting families and building a strong community network. #J-18808-Ljbffr CAMBA
