University of Colorado Anschutz Medical Campus Department: Pediatrics, Section of Palliative Medicine Job Title: Instructor or Senior Instructor (Open rank) Working Title: Advanced Practice Provider (APP)(PA/NP/APRN) Position #: 00 850530– Requisition#:40589 Job Summary: This position is integral to the growth of the Pediatric PalliativeMedicine Section at Children's Hospital Colorado, which supportspatients with life-altering and/or life-threatening conditions.This position will work with a diverse interdisciplinary group ofclinicians and support team members to deliver pediatric palliativecare through pain and symptom management, assistance withclarification of goals of care, spiritual support, psychosocialsupport, and emotional support. The position will work primarily inthe inpatient setting as well as across the continuum of careincluding outpatient and community teams, and will collaborate withconsulting services, facilitating a trans-disciplinary approach tocare. Key Responsibilities: Conduct comprehensive, focused assessments of children and families with palliative care needs. Formulate, implement, monitor, evaluate and modify palliativecare plans and program activities with input from palliative careteam and other collaborating care providers including communityprograms and facilities. Work with patients and families to determine their understandingof their child's condition, proposed treatment plan and treatmentoptions. Assist children and families to develop self-care capabilitiesrelative to identified problems. Assist in all facets of care coordination for palliative carereferrals. Engage in quality improvement in patient care services anddepartment functions, including evaluating the effectiveness of thehealth care delivery system in meeting the health care needs ofpediatric palliative care patients and families. Advocate for improvements in pediatric palliative care throughparticipation in professional, private, and governmentalgroups. Provide clinical expertise and guidance for undergraduate and/orgraduate students in the specialty area. Act as resource person for clinical information and education asrequested by community agencies. Assess the environment for potential need for consultation. Plan clinical and educational interventions which are consistentwith individual/organizational needs and goals and evaluatechange/outcomes. Implement patient and family education in collaboration with theinterdisciplinary team. Participate in pediatric palliative care research, includingquality and performance improvement initiatives. Define goals for professional growth and participate actively inprofessional activities and organizations. Maintain continuous professional certifications and licensureincluding continuing education activities. Demonstrate accountability for evaluation of clinical practice,including adherence to organizational, state and national practicestandards and codes. Engage in active and frequent self-care activities for personaland professional growth and longevity. Rotating weekend and evening call coverage either in person or bytelephone. Master's Degree leading to ability to be certified an AdvancedPractice Nurse (Pediatric or Family or Physician Assistant. The applicant must be licensed to practice as an AdvancedPractice Nurse or Physician Assistant in the State of Colorado bydate of hire. Current DEA license in the State of Colorado or ability to obtainone by date of hire. Sr. Instructor: Minimum of 8 years of clinical experience, including care ofchildren with complex, chronic and/or life- threatening conditions,or those at end of life. Master's Degree and certified as an Advanced Practice Nurse(Pediatric or Family or Physician Assistant. The applicant must be licensed to practice as an AdvancedPractice Nurse or Physician Assistant in the State ofColorado. Preferred Qualifications: Hospice and Palliative Care Certification and/or prior trainingin serious illness communication or pediatric painmanagement. Clinical nursing care or physician assistant experience inhospice and palliative medicine, children and youth with specialhealth care needs (CYSHCN), or pediatric/neonatal intensivecare. Bi-lingual and Spanish-speaking Knowledge, Skills and Abilities: Ability to effectively communicate within a multidisciplinaryteam. Strong active listening skills when communicating with patientsand families, staff and other professionals. Robust capacity to work in a team environment. Strong organizational skills. Strong advocacy skills. Knowledge of palliative care concepts including, but not limitedto: symptom management, advanced care planning, spiritual supportand psychosocial support. Basic awareness of government waivers for palliative and hospicecare. Knowledge of medications and prescriptive authority per statelaw. Anticipated Pay Range: The starting salary range (or hiring range) for this position hasbeen established as HIRING RANGE: Instructor: $111,000- $127,000 Senior Instructor: $127,000- $133,000 The above salary range (or hiring range) represents theUniversity's good faith and reasonable estimate of the range ofpossible compensation at the time of posting.
