The Department of Health and Social Services (DHSS) in Delaware seeks a Health/Human Service Case Manager II to determine initial and ongoing eligibility for needs-based benefits, including SNAP, Medical Assistance, CHIP, Subsidized Childcare, GA, and TANF. You will work with TANF and SNAP vendors, provide case management, and refer clients to additional services using a trauma-informed approach. As part of a multidisciplinary team, you will connect vulnerable residents with resources, maintain #J-18808-Ljbffr State of Delaware
