Requirements:

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the state of practice.
• Experience in skilled nursing facility or geriatric rehabilitation preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work both independently and as part of an interdisciplinary team.

Roles & Responsibilities:

• Perform initial and ongoing assessments of residents' mobility, balance, strength, gait, and functional limitations.
• Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability.
• Provide direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and the use of physical agents or modalities.
• Monitor residents' progress and adjust care plans as necessary to ensure achievement of therapy goals.
• Participate in interdisciplinary care team meetings and contribute to discharge planning as appropriate.
• Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence.
• Supervise and guide Physical Therapist Assistants and therapy aides in accordance with licensure requirements.
• Accurately document treatment sessions, progress notes, and billing information in a timely manner.
• Ensure compliance with all state, federal, and facility documentation and practice standards.
• Support quality assurance and performance improvement initiatives as part of the rehabilitation team.
