• Provide comprehensive outpatient care to adult and senior patients
\n
• Diagnose and manage complex medical conditions
\n
• Maintain accurate documentation and coordinate necessary referrals
\n
• Collaborate with an interdisciplinary care team
\n
• Participate in quality improvement and clinical initiatives
\n

\n
• MD or DO; active, unrestricted medical license
\n
• Board Certified in Family Medicine, Internal Medicine, or Geriatric Medicine
\n
• Minimum 2 years of experience in value‑based care or high‑acuity geriatric populations
\n
• Commitment to patient‑centered, outcomes‑driven care
\n
• Participation in TB screening
\n

• Training in Family Medicine, Internal Medicine, Med‑Peds, or Geriatrics
\n
• Experience in senior care or value‑based care settings
\n
• Proficiency with EHR systems
\n
• Strong communication and team collaboration skills
\n
• Experience supervising NPs/PAs
\n

• Clinic Hours are Monday-Friday (8a - 5p), this position is 32 hours a week
\n
