The ideal candidate brings clinical excellence, strong relationship-building skills, and a genuine commitment to enhancing the quality of life for the senior population we serve.Responsibilities include but are not limited to:Analyze and interpret patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnosesBuild a rapport with the patient and explain condition and course of action effectively through written and oral communicationDevelop treatment plans based on scientific rationale, standards of care, and professional practice guidelinesPromote preventive care and health maintenance including annual physicals, positive health behaviors and self-care through both formal and individual education and counselingMaintain complete and detailed records of patients' health care plans and prognosesPerform quality control and maintain a high standard in relation to infection control mechanisms for patient safetyWork collaboratively with the multi-disciplinary health care team to provide the best outcomes for patientsOffers of employment are contingent upon passing a background check and drug screen.Legacy Medical is a team of nurse practitioners and physicians providing on-site primary care to residents in senior living communities in North Dakota, South Dakota, Minnesota, Wyoming, and now Arizona.
