Work Environment

This role is primarily onsite, allowing the NP to interact directly with patients in their homes or in our designated concierge facilities. With a focus on work-life balance, our typical hours are Monday through Friday, from 9 AM to 5 PM. You will work closely with a team of dedicated healthcare professionals in a supportive and innovative environment, using state-of-the-art electronic health record systems and clinical workflows tailored for optimal patient care.

Key Responsibilities

• Conduct comprehensive patient assessments, including detailed medical history reviews, physical examinations, and health screenings tailored to individual patient needs.
• Diagnose a wide range of acute and chronic health conditions, and develop treatment plans that include medication prescribing and ongoing management strategies.
• Create and implement individualized care plans with an emphasis on preventive healthcare, lifestyle modifications, and patient education.
• Perform minor procedures such as wound care, suturing, and injections, ensuring patient comfort and safety.
• Order and interpret diagnostic laboratory tests and imaging studies, effectively communicating results to patients and their families.
• Engage in collaborative practices with physicians and specialists to provide holistic care and care coordination.
• Empower patients through health education on condition management and lifestyle changes to enhance their overall well-being.
• Maintain accurate and thorough patient documentation in compliance with HIPAA regulations and organizational standards.

Qualifications

Compensation and Benefits
