Key Responsibilities

• Conduct comprehensive health assessments and develop personalized care plans for patients in a variety of settings, including their homes.
• Utilize established standardized protocols to diagnose and manage acute and chronic health conditions.
• Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to provide holistic patient care.
• Educate patients and their families on health promotion, disease prevention, and self-management of conditions.
• Document patient information accurately in electronic health records (EHR) systems to ensure continuity of care.

Patient Population / Client Focus

Qualifications

Compensation and Benefits
