Collect ongoing data on residents’ physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team. Contribute to the development and execution of an age‑appropriate, culturally relevant plan of care with residents and families, and implement nursing interventions. Reassess and revise the plan of care as resident outcomes are evaluated; initiate referrals as needed. Maintain professional knowledge and participate in continuing education and in‑service activities; mentor and orient other staff as appropriate. Maintain safety and infection control practices; support a collaborative team environment with physicians and ancillary staff. Demonstrate competency in geriatric and adult care, ensuring care is aligned with resident goals and age‑specific needs. Requirements Active New York LPN license. BLS certification (American Heart Association or RQI). Minimum 1 year of LPN experience, preferably in long‑term care. Experience providing care to geriatric residents. Ability to work evenings and participate in rotating shifts (D/E/N/W as applicable). Demonstrated ability to function under supervision of an RN and collaborate with the interdisciplinary team.
