Recommend revisions to the plan of care after evaluating resident outcomes; arrange referrals as needed. Collaborate with residents, families, and the Interdisciplinary Team to ensure comprehensive care and a safe environment. Maintain current nursing knowledge and competency; participate in mentoring and orientation of staff; provide leadership under the RN. Apply standard precautions and maintain safety when caring for adults and geriatric residents. Requirements Active NY LPN licensure. BLS certification (American Heart Association or RQI). Minimum 1 year of LPN experience, preferably in a long‑term care setting. Ability to provide direct and indirect skilled nursing care to adult and geriatric residents. Comfortable working under RN supervision and collaborating with an Interdisciplinary Team. Capacity to work a 5x8 rotating schedule, including weekends and holidays.
