JOB SUMMARY: 

The Host Home Case Manager is responsible for overseeing the quality, compliance, and continuity of care provided to individuals placed in host home settings. This role manages the full case management lifecycle — from ISP implementation and documentation oversight to home provider training, supervision, and recruitment —ensuring services meet agency standards and DBHDD requirements. The ideal candidate is detail-oriented, comfortable with heavy documentation and compliance tracking, and confident conducting in-home visits and working directly with individuals, families, and home providers.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Work closely with individuals to ensure Individual Service Plans (ISPs) are reviewed, implemented, and followed effectively.

• Monitor implementation of individual goals, supervision requirements, person-centered preferences, and desired outcomes.

• Review Therap documentation, including shift notes, goal tracking, and medical/health tracking

• Attend ISP, interagency, DBHDD, and other meetings as requested.

• Communicate with individuals' families and natural support at least quarterly

• Review and monitor Therap documentation for completeness, accuracy, and compliance

• Electronically file individual documentation in eBridge.

• Review medical visit records and route completed records to the assigned agency nurse and coordinator.

• Upload and maintain required documents in Therap (appointments, records, etc.)

• Ensure home providers receive updated HRSTs, ISPs, assessments, and other required records.

• Serve as designated reviewer for financial records, attendance calendars, medical visit records, evacuation drills, and other required forms.

• Ensure all monthly paperwork is submitted by the 5th of each month.

• Review submissions for accuracy, completeness, and compliance.

• Identify and resolve missing, incomplete, or inaccurate documentation with home providers.

• Review, approve, and submit home provider invoices for monthly reimbursement.

• Monitor Relias training records to ensure home providers stay compliant with agency/DBHDD standards.

• Complete assigned training and documentation by required deadlines.

• Conduct additional training on individual-specific needs, provider enrichment, and DBHDD-mandated hours.

• Provide ongoing guidance to home providers on training and documentation expectations.

• Conduct monthly field-day home visits to monitor quality of services.

• Observe individuals in-home to assess well-being, needs, and ISP implementation.

• Review home provider documentation for accuracy during visits.

• Conduct site inspections against operational standards (per Attachment A: Operational Standards for Host Homes Manual).

• Conduct pre-placement inspections for new residences as needed.

• Provide ongoing support and supervision to home providers.

• Monitor provider performance and compliance with agency/DBHDD standards.

• Complete annual host home assessments and provider evaluations.

• Identify improvement areas and support provider development.

• Communicate with prospective home providers and share application information.

• Coordinate completion/submission of host home applications and agency documentation.

• Assist with preparing and reviewing documentation for prospective providers through placement.

EXPERIENCE: 

• Two years’ experience working with individuals with intellectual and developmental disabilities

• Minimum two years’ experience working with individuals with intellectual and developmental

disabilities

• Comfortable with consistent travel throughout the state of Georgia

SPECIAL LICENSES, REGISTRATION OR CERTIFICATION:

• Bachelor’s degree in human services or related field
