Population Served: Adult women, including pregnant and postpartum women, and women residing with their children

Position Overview

The Case Manager provides comprehensive, participant-centered case management services to women receiving residential substance use and opioid use disorder treatment, including mothers residing in the program with their children. The Case Manager supports participants in addressing barriers that may affect treatment engagement, recovery, family stability, and successful transition to independent living.

The Case Manager works collaboratively with participants, clinicians, peer support staff, medical providers, childcare staff, housing providers, community agencies, and other members of the interdisciplinary treatment team. The position focuses on helping participants access and coordinate services related to housing, transportation, benefits, employment, education, childcare, healthcare, legal needs, identification documents, financial stability, and other recovery-supportive resources.

The Case Manager promotes self-sufficiency, family preservation, participant choice, trauma-informed care, and successful discharge planning.

Essential Job Responsibilities

Participant Assessment and Case Management

· Complete case management assessments to include but not limited to Work Readiness Assessments, Recovery Capital Assessments and identify participant strengths, needs, barriers, and available resources.

· Assist participants in developing individualized service plans within first few days of admission, and follow up ISPs in 30, 60, 90 and 180 days tracked by admission date on EMR dashboard.

· Provide ongoing case management services throughout the participant's residential stay.

· Maintain appropriate boundaries and a professional helping relationship with participants.

· Advocate for participant needs while promoting independence and self-advocacy.

· Assist participants in identifying and utilizing natural supports and community resources.

· Provide culturally responsive, trauma-informed, recovery-oriented services.

Housing and Independent Living

· Assess housing needs early in the participant's treatment episode.

· Assist participants with identifying safe, stable, and recovery-supportive housing.

· Provide referrals to transitional housing, sober living, permanent supportive housing, shelters, and other appropriate housing resources.

· Assist participants with housing applications, documentation, interviews, and follow-up.

· Coordinate with housing providers and community partners as appropriate.

· Help participants address barriers such as prior evictions, lack of income, identification documents, credit issues, and transportation.

· Develop housing transition plans before discharge.

· Track housing outcomes following treatment when required by program or grant requirements.

Benefits and Basic Needs

Assist participants in accessing and maintaining appropriate benefits and resources, including:

· Medicaid/health insurance

· SNAP/food assistance

· TANF and other public benefits

· Social Security benefits when applicable

· Identification cards and birth certificates

· Social Security cards

· Transportation assistance

· Clothing and personal care resources

· Food and household resources

· Community assistance programs

The Case Manager will assist participants with applications and documentation but will promote participant independence rather than completing tasks that the participant can reasonably complete herself.

Employment, Education, and Financial Stability

· Assess employment, education, vocational, and financial needs.

· Assist participants with obtaining identification, birth certificates and employment documentation, as needed.

Transportation and Community Resources

· Assist participants in developing reliable transportation plans before discharge.

· Provide information regarding public transportation, transportation assistance programs, rideshare resources, and community transportation services.

· Assist participants in obtaining driver's licenses or state identification when appropriate.

· Develop individualized transportation plans for employment, healthcare, childcare, education, recovery meetings, and other essential activities.

Legal and Social Service Coordination

· Assist participants in identifying appropriate legal and social service resources.

· Provide referrals for legal aid, family court, child support, custody, driver's license reinstatement, and other legal needs as appropriate.

· Coordinate with probation/parole or other justice-system entities when authorized and clinically appropriate.

· Maintain appropriate documentation of referrals and participant follow-up.

Interdisciplinary Team Responsibilities

The Case Manager will:

· Coordinate services with counselors, clinicians, peer support specialists, medical staff, childcare staff, housing staff, career-development staff, and other program personnel as needed.

· Identify changes in participant functioning or circumstances that may require clinical or peer support interventions.

· Make appropriate referrals to clinical staff when a participant presents with needs outside the Case Manager's scope of practice.

· Maintain clear role boundaries between case management, clinical counseling, peer support, and other disciplines.

Documentation Responsibilities

The Case Manager is responsible for timely, accurate, and clinically appropriate documentation, including:

· Case management assessments

· Individualized service plans

· Case management progress notes

· Referral documentation

· Appointment documentation

· Benefit applications and outcomes

· Community resource referrals

· Follow-up documentation

· Other documentation required by organizational policy, funding sources, licensing bodies, or accreditation standards

Documentation must be completed within established organizational timeframes and maintained in accordance with confidentiality requirements.

Requirements

Minimum Qualifications

· Bachelor's degree (B. A.) from four-year college or university; or three to four years related experience and/or training; or equivalent combination of education and experience.

· Department of Family and Children Services experience preferred
