Intensive Case Manager - Homeless Services & Support

Linc Housing Corporation is seeking an Intensive Case Manager in Willowbrook to join the Intensive Case Management Services team. You will coordinate services for formerly homeless individuals with chronic conditions, using motivational interviewing, harm reduction and trauma-informed care. Reports to the Senior Case Manager of ICMS and collaborates with partners.
