About the Role:

We are seeking a dedicated and compassionate Weekend Hospice Registered Nurse to join our team in Chicopee, Massachusetts. As a member of our team, you will provide exceptional patient care and support to our patients and their families on weekends and during on-call rotations. Responsibilities:

• Provide skilled nursing care to patients in their homes, hospitals, and hospice facilities

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Conduct patient assessments, identify needs, and coordinate services to ensure continuity of care

• Communicate effectively with patients, families, and healthcare providers to ensure timely and effective treatment

• Providing 24/7 support to patients and families

• Collaborate with the healthcare team to ensure seamless transitions and quality care
