Key Responsibilities\n

• Maintain accurate and timely clinical documentation, including comprehensive progress notes and treatment records, in accordance with organizational policies.

• Collaborate with a multidisciplinary healthcare team to craft patient-centered treatment plans and discharge strategies informed by thorough psychosocial assessments.

• Engage actively with clients’ families and support systems to coordinate care and ensure alignment in treatment goals.

• Develop and implement individualized treatment plans tailored to meet the diverse needs of each patient.

• Conduct regular counseling sessions, utilizing evidence-based therapeutic modalities, and adjust treatment approaches based on ongoing patient evaluations and feedback.

Patient Population / Client Focus\n

The social worker will primarily support students enrolled in our partner schools, addressing a range of issues such as trauma, anxiety, depression, and behavioral challenges. Qualifications\n

• A Master’s degree in Social Work, Counseling, or a related field is required.

Compensation and Benefits\n
