RN Home Health Case Manager Position Responsibilities:
• Conduct comprehensive nursing assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and evaluate individualized care plans in coordination with the interdisciplinary team (including physicians, therapists, social workers, and aides).
• Provide direct skilled nursing care, such as wound care, medication management, IV therapy, symptom management (especially pain and comfort in hospice), and patient/caregiver education.
• Offer education and emotional support to patients and their families/caregivers based on individual needs.
• Supervise LPNs/LVNs, home health aides, and other support staff.
• Coordinate care activities, including visit scheduling and documentation to ensure compliance with regulations.
• Monitor patient progress, respond to changes in condition, and adjust care plans accordingly. Requirements

- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse
- One (1) year experience as a RN.
- Must possess and maintain valid CPR certification while employed in a clinical role.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Ability to travel to all business locations.
