Key Responsibilities\n

• Conduct comprehensive and ongoing assessments to determine the mental health needs of clients.

• Facilitate communication among supervisors, direct care staff, families, and other relevant parties regarding patient progress and care plans.
\n
• Develop and provide a schedule of appointments and home visits to supervisors for monitoring client care and ensuring adherence to the treatment plan.
\n
• Coordinate and perform home visits as needed, ensuring thorough evaluations, charting, and data analysis.
\n
• Oversee and ensure the effective implementation of individualized treatment plans.
\n
• Actively participate in Interdisciplinary Team Meetings to discuss patient progress and collaborate on treatment strategies.
\n
• Create and maintain detailed progress notes for clients on a monthly and quarterly basis.
\n
• Organize and conduct training sessions to ensure staff competency in program implementation and documentation practices.
\n
• Provide ongoing follow-up through interviews, home visits, and observations to ensure continuous support for client needs.
\n
• Maintain accurate records to ensure timely access to necessary information by relevant stakeholders.
\n
• Serve as a liaison between clients and the broader mental health community to support development goals.

• Contribute to the creation of training materials aimed at enhancing the delivery of services and programs.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.

• Proficient understanding of legal and ethical standards of mental health practice.

• Excellent interpersonal and communication skills, with the ability to establish rapport with a wide range of clientele.

• Demonstrated ability to work collaboratively within a healthcare team environment.
