ER Care Manager RN: Coordinated Patient Care & Transitions

LCMC Health seeks a Care Manager who acts as the hub of patient case management and care planning. You will assure quality across the continuum, collaborate with the care team, and implement care plans and transition strategies to achieve optimal outcomes. Weekends are required in the United States. A Louisiana nursing license and 2 years of professional nursing or care management experience are preferred.
