Population Health Outreach Specialist

Collaborating with internal and external stakeholders, the full-time Population Health Outreach Specialist will optimize health management for attributed patients, focusing on primary care and supporting value-based payer contracts while working onsite with a hybrid option after 90 days. Key responsibilities Engage proactively with patients to improve healthcare delivery and outcomes Support the Care Coordination Program across all SLPG Primary Care providers and clinics Collaborate with stakeholders to maximize performance within value-based contracts Required qualifications Less than 1 year of experience as a Medical Assistant (Certified) Associate Degree in a relevant field Understanding of population health management and value-based care principles
