Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians to devise personalized treatment plans, focusing on achieving measurable improvements in function and mobility.
• Administer therapeutic interventions using established techniques such as manual therapy, modalities, and exercise programming while adhering to patients' specific rehabilitation goals.
• Effectively communicate with patients to inform them about their treatment plans, expectations, and progress, fostering a supportive relationship that encourages compliance and motivation.
• Collaborate with other healthcare team members to optimize the delivery of care, ensuring an interdisciplinary approach to treatment and monitoring progress toward shared goals.
• Educate patients and their caregivers about exercises, postures, and lifestyle adjustments that can aid in recovery and enhance long-term health outcomes.
• Regularly document patient progress and treatment outcomes in compliance with organizational protocols, using electronic health records (EHR) to facilitate continuity of care.
• Participate in community outreach programs that promote healthy living and raise awareness of physical therapy benefits, thus strengthening community ties.

Patient Population / Client Focus

As a Physical Therapist at Agile Body Therapy & Home Care, you will work with a varied patient demographic, including individuals recovering from orthopedic injuries, pediatric patients with developmental conditions, neurologic patients, and athletes requiring sports rehabilitation. Your role will focus on building therapeutic relationships that empower patients and motivate them to take an active role in their recovery process.

Qualifications

• A Bachelor's degree in Physical Therapy from an APTA or CAPTE-accredited program.
• Current state licensure in Physical Therapy, ensuring compliance with all local regulations.
• Basic Life Support (BLS) certification from the American Heart Association, demonstrating your commitment to patient safety and emergency preparedness.
• Exceptional communication skills, both written and verbal, allowing you to effectively engage with patients and their families.
• Proficiency in using electronic health records (EHR) and other computer systems for documentation and communication purposes.
• A demonstrated ability to build rapport with patients and their families, embodying compassion and professionalism in every interaction.

Preferred Experience

• Experience in treating a wide array of conditions across different patient age groups, particularly in a rehabilitation setting.
• Knowledge of evidence-based practices and familiarity with the latest rehabilitation technologies and methodologies.
