Responsibilities Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.

- Orchestrate clinical communication across the multidisciplinary team to ensure the health plan is executed seamlessly and no detail is overlooked.
- Champion documentation integrity by serving as a clinical resource, ensuring medical records accurately reflect the necessity of services and the complexity of the patient's journey.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
