URGENT HIRE: Full-Time Hospice RN Case Manager (RNCM)

Castle Hospice, a part of Castle Senior Living is seeking a compassionate, dedicated Registered Nurse Case Manager (RNCM) to join our team. This full-time role provides care to patients in Racine, South Milwaukee, and surrounding communities.

Position Summary

As a Hospice RN Case Manager, you will provide skilled, patient-centered nursing care to individuals and families in their homes. You will: Perform comprehensive physical, emotional, social, and spiritual assessments Develop and implement individualized care plans Deliver skilled nursing interventions aligned with hospice standards Collaborate with the Hospice Director of Nursing to support 24/7 care coverage Participate in an on-call rotation (with reduced on-call as staffing grows)

What We're Looking For

• Current Registered Nurse (RN) license in Wisconsin
• 1+ year of experience in medical/surgical, hospice, home health, or skilled nursing (preferred)
• Excellent communication and problem-solving abilities
• Compassionate bedside manner and a heart for hospice care
• Valid driver's license + current auto insurance
