Hospice RN Case Manager

We are seeking a compassionate and experienced RN Case Manager to join our hospice and palliative care team. This role is responsible for coordinating and delivering patient-centered care to individuals with life-limiting illnesses, ensuring comfort, dignity, and quality of life for patients and their families.
KEY RESPONSIBILITIES• Maintain the individualized plan of care and update with every significant change in condition.
• Execute symptom-management pathways and standing orders; lead change-in-condition response.
• Provide proactive family education and closed-loop communication with community staff. Required Skills & Experience
- Active WI RN license as assigned); BSN preferred.
- 1+ year hospice/home-health or relevant clinical experience; CHPN preferred.
- Caseload management within 12-15 target; comfort with after-hours rotation.
- Knowledge of §418 CoPs and Wisconsin CBRF/RCAC medication rules where applicable.
Nice to Have Skills & Experience
- Experience in the assisted living space
- Experience With Homecare Homebase
