Program Description

The Triage, Consultation, and Wellness Response Team (TCWRT) is a Felton Institute program funded by the Alameda County Probation Department (ACPD) and the Alameda County Behavioral Health Department (ACBHD) under the AB 109 Mental Health and Wellness (MHW) initiative. TCWRT serves AB 109-eligible clients - including individuals released from Santa Rita Jail or CDCR facilities, and those on Post-Release Community Supervision, formal probation, or in specialty court programs - by connecting them to mental health, substance use, and wellness services. The team provides clinical consultation and triage, peer and supportive services, wellness response and de-escalation, outreach and mental health promotion, and administration of the Specialty Provider Pool (SPP). TCWRT staff are stationed across the Oakland and Hayward ACPD offices, the Center of Reentry Excellence (CORE), and AB 109 Housing Program locations, and the program operates under a detailed set of contractual reporting, data, and fiscal compliance requirements.

Position Summary

The Program Manager provides clinical and operational leadership of the TCWRT, ensuring all contractual deliverables, quality objectives, and impact targets are met. This role supervises a multidisciplinary team of 5.50 FTE and serves as Felton Institute's principal liaison to ACPD, ACBHD, Deputy Probation Officers (DPOs), community-based partners, and the broader Alameda County reentry system. The Program Manager is accountable for clinical integrity, programmatic performance, regulatory compliance, fiscal stewardship, and community impact across multiple service sites.

Key Responsibilities

• Direct day-to-day clinical and administrative operations, ensuring services align with AB 109 goals: connecting clients to mental health and prevention services, improving wellbeing, promoting desistance, preventing recidivism, and supporting diversion into least-restrictive settings.
• Provide clinical supervision, case consultation, and triage oversight using evidence-based practices (EBPs) including Contingency Management, Motivational Interviewing, peer support models, and strength-based strategies.
• Ensure Provider Service Plans (PSPs) are developed within 30 days of active engagement for 100% of AB 109 clients, and that 100% of active clients are triaged to a community provider within 60 days of referral.
• Lead the Wellness Response function, ensuring on-site arrival within 40 minutes of a mental health incident at ACPD offices, CORE sites, and AB 109 Housing Programs: coordinate with law enforcement and County crisis teams, including 5150 recommendations as appropriate.
• Oversee field staffing schedules across all service locations

Specialty Provider Pool (SPP) Administration

• Administer the SPP, including provider recruitment, contracting, oversight, and payment management for a culturally responsive, unlimited pool of providers.
• Convene and facilitate the SPP selection panel (ACPD, ACBHD, TCWRT, and community representatives) to vet and approve providers.
• Ensure SPP referrals receive a response within 24 hours, and client outreach begins within 72 hours; monitor the SPP fund balance and submit monthly reports on activity, client progress, and outcomes.

Team Supervision & Staff Development

• Supervise and develop 5.50 FTE direct reports: 2.0 FTE Clinical Case Managers (one bilingual), 2.0 FTE Peer Support Specialists, 0.50 FTE Prescribing Practitioner, and 1.0 FTE Quality Data Manager.
• Recruit, hire, train, and retain staff, ensuring at least 30% are formerly incarcerated (with formerly incarcerated representation in both upper and middle management), 100% of formerly incarcerated staff earn a living wage, and at least one bilingual staff member is on the team.
• Coach staff on trauma-responsive approaches, working with clients with Serious Mental Illness (SMI), Motivational Interviewing, Contingency Management, harm reduction, and culturally responsive practice.
• Ensure all required and County-requested trainings are completed.

Contract Compliance, Reporting & Quality Assurance

• Ensure full compliance with the AB 109 MHW Scope of Work: minimum 100 clients served annually, 5 hours of weekly service per site, 10 resource-sharing activities, client celebration events, and community reentry workshops.
• Drive quality objectives: outreach to 100% of referred clients; contact with 70% within 72 hours; 50% enrollment; 100% referrer response within 24 business hours; and contact with 70% of pre-release clients within 72 hours of release.
• Drive impact objectives: 70% of families report increased confidence; 70% of clients complete the program with PSP goals met; 70% continue independent participation 30 days after warm hand-off; at least 25% of services utilize EBPs.
• Lead development and submission of the TCWRT Referral Plan within 30 days of contract start.
• Oversee timely data entry into the ACPD Provider Portal (within 72 business hours of service delivery) and submit all required monthly, quarterly, annual, and final reports per ACBHD timelines and templates.
• Report adverse client events (within 24 hours), staff changes (within 1 business day), and release-of-information refusals (within 48 hours) to ACPD.

Direct Service & Wellness Practice

• Provide direct consultation to DPOs and AB 109 clients on mental health navigation, utilizing the MHS 140 Intake Form, SHIE Community Health Record, and other authorized data sources.
• Support assessment and information gathering, including collection of health records from in-custody, hospital, and community providers (with client consent).
• Participate in case planning, care coordination, and discharge planning with ACPD, in-custody providers, psychiatric hospitals, crisis residential programs, ACBHD, and mental health and substance use providers.
• Oversee ACCESS referrals and ensure case management continuity (e.g., two months of dual services) until a stable provider connection is established.

Minimum Qualifications

• Master's degree or higher in Social Work, Psychology, Counseling, Marriage and Family Therapy, Public Health, Public Administration, or a closely related behavioral health or human services field.
• 5+ years of progressively responsible experience in behavioral health, social services, or a related human services field.
• 3+ years of supervisory or management experience leading multidisciplinary teams that include clinical, peer, prescribing, and/or data/quality assurance staff.
• Demonstrated experience serving justice-involved adults (PRCS, PC 1170(h), formal probation, pre-trial, and specialty court populations).
• Demonstrated experience administering government contracts, meeting county or state deliverables, and managing data reporting through provider portals.

Knowledge, Skills & Abilities

• Working knowledge of Alameda County mental health, substance use, and crisis systems, including ACCESS, ACBHD networks, Medi-Cal, managed care plans, and community wellness resources.
• Competency in EBPs: Motivational Interviewing, Contingency Management, peer support models, trauma-informed care, harm reduction, and strength-based strategies.
• Strong knowledge of HIPAA, 42 CFR Part 2, and release-of-information requirements.
• Skilled in crisis assessment, de-escalation, and the appropriate use of 5150 procedures.
• Ability to write clear, plain-language client documents (e.g., PSPs) and contract-aligned reports.
• Excellent organizational, written, and verbal communication skills; ability to present effectively to diverse audiences..
• Valid California driver's license, reliable transportation, and proof of insurance; ability to travel throughout Alameda County and work occasional evenings and Saturdays.
