Telehealth Care Coordinator

Supporting clients throughout their treatment journey, the full-time Telehealth Care Coordinator will manage client intakes, care coordination, and communication with multidisciplinary teams while working remotely. Key Responsibilities Conduct comprehensive telehealth intake assessments and coordinate with internal teams for smooth treatment initiation Provide education and support to clients, conducting check-ins to monitor progress and reinforce treatment goals Maintain accurate documentation and ensure compliance with organizational policies and regulatory standards Required Qualifications Associate's degree or higher in Social Work, Psychology, or a related Human Services field Two to three years of experience in behavioral health, mental health, or substance use disorder treatment settings Knowledge of state and federal confidentiality regulations and Joint Commission standards Adherence to the Healthcare Code of Ethics and professional standards of conduct Ability to work effectively as part of a multidisciplinary treatment team
