Key Responsibilities Conduct thorough psychosocial assessments using interviews, direct observations, and reviews of medical records to evaluate emotional, social, and environmental factors that impact patient recovery. Provide therapeutic counseling and support to patients and their families, addressing adjustment issues, emotional challenges, and strategies for coping with disabilities. Collaborate with an interdisciplinary rehabilitation team to design and implement individualized treatment plans, incorporating patients' psychosocial needs and treatment goals. Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance. Educate patients and families about community resources available post-rehabilitation, ensuring they have the tools necessary for a successful transition back to daily life. Participate actively in discharge planning, facilitating smooth transitions for patients moving from our care to home or alternative care settings. Patient Population / Client Focus In your role, you will support a diverse range of patients, each facing unique challenges related to recovery from physical injuries, chronic illnesses, or neurological disorders. By integrating psychosocial perspectives into their rehabilitation, you help promote their emotional and mental wellness, crucial for effective recovery. Qualifications A Master’s degree in Social Work from an accredited program. Valid and active state licensure as a Licensed Clinical Social Worker (LCSW). Strong understanding of adjustment issues, grief, and effective coping strategies. Exceptional communication and interpersonal skills, enabling you to connect with patients and families meaningfully. Preferred Experience Previous experience in a rehabilitation or medical setting is highly preferred, lending insight into the unique needs of patients undergoing rehabilitation. Compensation and Benefits The salary for this position ranges from $75,000 to $100,000 annually, commensurate with experience and qualifications.
