Key Responsibilities\n

• Provide individualized therapy, counseling, and case management services to patients, helping them address mental health challenges and personal issues.

• Collaborate with clients to develop personalized treatment plans that are comprehensive, measurable, and tailored to their unique situations.

• Act as an advocate for clients, connecting them with essential resources such as financial assistance, housing programs, and other relevant social services.

• Deliver crisis intervention for clients facing acute mental health crises, including suicidal ideation, trauma responses, and substance abuse issues.

• Engage in community outreach and educational initiatives to promote mental health awareness and reduce stigma.

• Accurately maintain and update patient case records, treatment documents, and progress notes in alignment with HIPAA and applicable regulations.

• Participate in regular supervision and mentorship sessions aimed at fostering both clinical growth and personal development.

Patient Population / Client Focus\n

This role primarily serves a diverse clientele, including individuals, couples, and families experiencing a range of emotional and psychological challenges. The population served may also include patients within a hospice setting, where providers are focused on delivering not only mental health support but also end-of-life care that respects the dignity and emotional needs of patients and families.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• Possession of a valid New Jersey LCSW license, enabling independent practice.

• Demonstrated experience engaging with individuals, couples, and families in a therapeutic context.
