As a Home Health Registered Nurse (RN), you will work closely with a team to develop treatment plans for patients. You will also have the opportunity to creatively utilize resources available in their homes, educate patients and their families, and adapt care strategies to meet the changing needs of your patients.
Service AreasThis role covers the following counties in Illinois: Clinton, Washington, Perry, Marion, Jefferson, Franklin, Wayne, Hamilton, and Edwards.
CompensationPay Range: $32.77 - $52.43 per hour
Compensation is determined by various factors, including relevant experience, labor market conditions, and other qualifications. The stated pay range reflects what we reasonably expect to offer at the time of posting, but actual pay may vary in accordance with applicable laws.
Key Responsibilities- Evaluate and coordinate patient discharge planning needs with the healthcare team.
- Conduct point-of-care testing as per established policies.
- Collaborate with physicians regarding care plans and patient status updates, documenting all communications in compliance with regulations.
- Engage with multidisciplinary team members and external agencies to ensure effective patient care and education throughout the discharge process or end-of-life care.
- Complete admission documentation and verify insurance details, ensuring timely medical assessments are recorded in the clinical system.
- Deliver skilled nursing care as outlined in the care plan, including vital signs monitoring, wound care, IV therapy, ostomy care, blood draws, injections, and comprehensive patient assessments.
- Document visit notes, treatments, and care accurately and promptly, adhering to physician orders and regulatory requirements.
- Work collaboratively with patients, their families, and the healthcare team to create a tailored care plan that addresses individual patient needs.
- Respond to inquiries from patients and families, providing follow-up to resolve any concerns.
- Maintain a state of alertness and safety while performing duties.
- Carry out additional responsibilities as assigned.
- Education: Graduate from an accredited nursing program or equivalent educational background for licensing.
- Experience: No prior experience is necessary.
- Additional Requirements: Proof of motor vehicle liability insurance that meets state minimums upon hire and upon request.
- Regularly communicate verbally to share information.
- Frequent standing and walking required.
- Occasional lifting and carrying of objects weighing up to 50 lbs.
- Frequent use of vision for near and far distances, as well as color differentiation.
- Regular driving and keyboard use for data entry.
- Occasional bending, stooping, and other physical movements.
For the state of Illinois:
- Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA)
- Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
- Valid Driver's License and Motor Vehicle Report (MVR)
For other states, similar certifications and licenses apply, including BLS HCP and RN credentials as per state regulations.
