Work Environment

Key Responsibilities

In your role as a Physical Therapist, you will:

• Design and implement individualized physical therapy treatment plans focused on restoring function and alleviating pain in line with the prescribed care plan.
• Evaluate each client's physical capabilities through comprehensive assessments, including muscle strength and joint mobility evaluations.
• Oversee and guide physical therapy assistants in executing care plans and ensure proper application of therapeutic techniques.
• Adapt treatment methodologies based on the clinical assessment to enhance clients' recovery and independence.
• Conduct home evaluations to recommend modifications that will enhance the client's capabilities and living conditions.
• Document patient progress meticulously, besides preparing in-depth clinical records, including initial evaluations, care plans, progress notes, and discharge summaries.
• Participate actively in interdisciplinary team meetings to discuss treatment strategies and optimize patient outcomes.
• Educate patients and families on therapy routines and proper use of assistive devices, ensuring they feel confident in supporting rehabilitation goals.
• Provide timely submissions of documentation and orders, maintaining adherence to branch policies and regulatory guidelines.

Patient Population / Client Focus

You will primarily work with a diverse patient demographic that includes elderly adults, individuals with post-surgical conditions, stroke recovery patients, and those with chronic illnesses. Each client will require tailored therapeutic strategies to support their individual rehabilitation journey, making adaptability and exceptional interpersonal skills essential for fostering a trusting therapeutic relationship.

Qualifications

The successful candidate will possess the following qualifications:

• Graduation from an accredited physical therapy program.
• Current PT licensure in the state of New Jersey, with a clean record.
• Current CPR certification to promote safety in clinical practices.
• A valid driver's license along with reliable automobile insurance.
• A strong commitment to clinical excellence and integrity in patient care.
• Excellent communication skills to engage effectively with patients, families, and team members.

Preferred Experience

While we require at least two years of experience as a Physical Therapist, candidates with prior experience in home care or acute care settings will be considered favorably. Familiarity with federal and state regulations regarding home healthcare is highly advantageous.
