Field Care Manager RN - Home Health & Wellness
Humana
Humana is hiring a Field Care Manager Nurse 2 to assess member needs, monitor progress, and coordinate home-based care with families and care teams. The role requires travel to member residences within 30–40 miles, strong communication, and the ability to work with minimal supervision. A valid driver’s license and auto insurance are required.
Preferred qualifications include a BSN and Case Management Certification, with additional experience in health promotion and care coordination.
