Arizona Licensed MTM Pharmacist

To support a dynamic clinical engagement team, the contract MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and document care delivered while working remotely with a flexible schedule. Key responsibilities: Engage members telephonically to schedule consultations and provide medication therapy management Utilize prescription claims data to generate clinical recommendations and assess medication regimens Document patient care notes and participate in departmental meetings to enhance clinical programs Required qualifications: Current and unrestricted Pharmacist license in the state of residence Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services 1+ years of experience working remotely in a cross-functional team environment Intermediate proficiency with Microsoft Word, Outlook, and Excel Bachelor's Degree or Pharm D
