You will educate providers and families, build referral relationships, coordinate admissions, and assess needs and barriers to care. This role combines relationship-building, clinical coordination, and sales strategy to improve patient outcomes. A few position highlights include:Educate health care professionals, patients, and families about home care and senior living services.Develop and maintain relationships with key referral sources, such as hospitals, nursing homes and physician offices to generate leads.Coordinates patient admissions with the team, including obtaining all required documentation to complete referral to admission process.Track leads, document activity in CRM, meet admissions and account goals, attend community events, and respond to customer concerns.THE MUST HAVES:At least two (2) years of experience in a healthcare related business development or community outreach role.
