PT, Skilled Nursing and Long Term Care

Physical Therapist

Exceptional opportunity for a PT with a passion for their craft to join an amazing organization. Healthcare system provides excellent care, is well established and has an excellent reputation in Skilled Nursing, Long Term Care facilities as well as Home Care. PT reports to the Clinic Director of Rehab and supervises PTA and support staff. PT coordinates patient care with PTA, OT, OTA and SLP members and rehab department. Treats residents, communicates with families, physicians, and other team members maintaining documentation for medical record.

Requires master's degree or doctorate degree in Physical Therapy. Licensed in the state of Ohio upon start date. Ideal candidate has experience within a SNF setting experience and willingness to learn Home Care. Welcome new graduates!

Compensation up to 115K depending on exp. Sign on of 10K with full medical benefits; vision, medical, dental and additional insurance with 401(k). PTO, Federal holidays and CME time with allowance. Cell phone, tablet and mileage reimbursement for home care visits.
