Lead Care Manager / Community Supports & Housing Navigation SpecialistPosition Type: 12-Month W2 Contract (renewal based on performance and program continuation) Schedule: Monday–Friday | 9:00 AM – 5:30 PM PST Compensation: $22–$24/hour + mileage reimbursement Work Model: Field-Forward (in-person visits required) Location Requirement: Must reside within Yuba CountyRole OverviewStar Nursing Services is seeking a Lead Care Manager / Community Supports Specialist to support CalAIM Enhanced Care Management (ECM) and Community Supports (CS) programs.You will serve as both a client-facing care coordinator and a field-based lead, supporting members with complex medical, behavioral, and social needs. This role focuses heavily on housing navigation, care coordination, and transitions of care, while also supporting team training, documentation quality, and program compliance.Your work directly impacts housing stability, health outcomes, and access to essential services.About CalAIM Community SupportsCalAIM Community Supports address social drivers of health by connecting members to critical services:Housing Navigation & DepositsHousing Tenancy & Sustaining Services (HTSS)Respite ServicesNursing Facility Transition / DiversionCommunity Transition ServicesPersonal Homemaker ServicesThese services reduce health disparities, improve care continuity, and support long-term stability.Key ResponsibilitiesClient Care & Field WorkConduct frequent in-person visits, assessments, and care plan updatesServe as the primary point of contact for assigned membersComplete intake, eligibility screening, and ongoing reassessmentsFacilitate care transitions across providers and settingsHousing Navigation & Community SupportsAssist members in locating and securing housingCoordinate housing deposits and move-in resourcesSupport housing tenancy and stabilizationConnect members to respite care and homemaker servicesIdentify and coordinate community-based resources (Section 8, 811, etc.)Care Coordination & CollaborationPartner with ECM teams, health plans, and community agenciesSupport members with chronic conditions (medical, behavioral, social)Coordinate services across healthcare, housing, and social systemsEscalate care needs and ensure timely interventionsLeadership & Program SupportTrain new staff on CalAIM workflows and county resourcesReview documentation for accuracy and compliance (MCP alignment)Support audits, corrective action plans (CAPs), and reporting requirementsRepresent Star Nursing in community meetings and partnershipsDocumentation & ComplianceMaintain accurate records in EMRs and required systemsComplete monthly reporting and data trackingEnsure HIPAA compliance and confidentiality standardsSupport audit readiness and quality improvement initiativesRequired QualificationsExperienceMinimum 1 year in case management, ECM/CS, housing navigation, or social servicesExperience with CalAIM, Medi-Cal, or managed care environments preferredKnowledge of authorizations, audits, grievances, and care coordination workflowsSkillsStrong communication and relationship-building skillsAbility to work independently in a field-based environmentOrganized, detail-oriented, and compliance-focusedCompassionate approach to high-risk populationsEducationAA/BA or equivalent experience in:Case ManagementSocial ServicesPublic HealthHousing NavigationHealthcare (CNA, MA, etc.)Ideal Candidate BackgroundExperience in the following areas is highly valued:CNA, PTA, or Medical Assistant rolesHuman Services or Community Support programsBehavioral Health or Psychiatric careCase Management or Social WorkPost-acute care settings (SNF, ALF, RCFE, Hospice, Home Health)Transportation RequirementsActive California driver’s licenseValid auto insurance and registrationReliable vehicle for county-wide travelTechnology RequirementsMicrosoft Office (Excel, Word, Teams, SharePoint)Adobe (PDF editing, e-sign tools)EMR systems for CalAIM documentationMinimum System Requirements:Windows 10+8GB RAM (16GB preferred)Intel i5 / Ryzen 5 or higher256GB SSD (No Chromebooks)Physical RequirementsPhysical or Medical clearence from DoctorAbility to travel frequently and perform field visitsSit, drive, walk stairs, and lift up to 30 lbsStrong verbal communication skillsPatient Rights & AdvocacyMaintain strict confidentiality of patient informationTreat all members with dignity, respect, and cultural sensitivityReport and address any concerns related to patient safety or rightsWhy This Role MattersYou support individuals facing housing instability, complex health needs, and systemic barriers.
