Center Clinical Director, Associate

The Clinical Director will directly supervise and train primary care providers (PCPs) in their assigned center, overseeing clinical quality, efficiency, outcomes, and clinician/patient satisfaction.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES

• Provide independent care for older adult patients with acute and chronic illnesses, taking full accountability for patient outcomes and seeking specialist consultation when necessary.
• Engage with hospitalists when patients are hospitalized, ensuring continuity of care regardless of affiliation.
• Conduct assessment, diagnosis, treatment, management, education, health promotion, coordination, and documentation for patients with complex chronic needs.
• Lead the care team—including medical assistants, facilitators, and coordinators—to deliver office-based care for patients who can visit.
• For patients unable to visit (hospital, SNF, LTC, or homebound), collaborate with transitional care teams, case managers, physicians, and other resources.
• Lead weekly Super Huddle (SH) and Transforming Care Meeting (TCM) sessions and support center clinical leadership when the Market Chief Medical Officer is unavailable.
• Actively monitor daily center census and perform joint accountability for outcomes.
• Support center operations, covering for providers as needed and participating in management decisions.
• Recruit patients for the center and interview, onboard, and mentor additional providers.
• Manage, mentor, and coach PCPs to deliver outstanding clinical outcomes and maintain engagement.
• Conduct leadership rounding with PCPs, providing feedback and guidance.
• Partner with Center Operations Director and Market General Manager to improve financial performance and increase center membership.
• Perform additional duties as assigned by management.

KNOWLEDGE, SKILLS, AND ABILITIES

• Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other company applications.
• Willingness to travel locally, nationally for initial training, and regionally/nationally for recruiting or training as needed.
• Fluency in English, verbal and written; fluency in additional languages may be required for certain centers and will be disclosed during application.
• Ability to exercise independent judgment in clinical and operational decisions.

EDUCATION AND EXPERIENCE

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
• Current, active MD licensure in the state of employment.
• Minimum one‑year clinical experience in a geriatric, adult, or family practice setting (preferred).
• Board certification in Internal Medicine, Family Medicine, Geriatrics, or a related specialty (preferred); board eligibility required.
• Current DEA number for schedule II‑V controlled substances.
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within the first 90 days of employment.

PAY RANGE

$238,832 – $341,189 per year (base salary). Final compensation depends on experience, education, geographic location, and other factors. The position may also be eligible for bonuses or commissions.
