Bilingual Patient Navigator - Brooklyn Care & Outreach
NYU Langone Hospitals
Family Health Centers at NYU Langone seeks a Patient Navigator to coordinate patient care, provide outreach and referrals, and support education and service plans. The role may include rapid HIV testing and counseling depending on grant requirements. The position requires a Bachelor's degree or equivalent with navigation experience, Spanish fluency preferred, and BLS certification at hire.
