Looking for a Full Time Licensed Social Worker for Navigator Homes

Navigator Homes on Martha's Vinyard provides all the tools necessary for you to excel in your role, including supportive management, good staffing levels, professional development, and an inclusive work environment.   This position reports to Alliance Health Executive Director. 

JOB SUMMARY

Assists in meeting the psychosocial needs of residents in a long-term care facility.  Collaborates with the inter-disciplinary team to develop and carry out individualized care plans to enable each resident to achieve their optimal level of health and well-being and maximize their quality of life.

QUALIFICTIONS

• MA LCSW required. 

• A minimum of 2-3 years' experience in a long-term care setting.

• Solid working knowledge of discharge planning, Medicare/Medicaid, OBRA regulations, community resources, social work methods and techniques.

• Excellent communication, interpersonal, and computer skills required.

• Knowledge of electronic medical records, MDS, and Care Plans.

Navigator Home of Marthas Vinyard is a  nursing home, offereing a warm, home-like enviornment where residents and staff thrive.  We are loooking for compassionate dedicated indivduals to join our team as Director of Nursing, leading and managing Universal Workers ,  in a unique role that goes beyone tranditional caregiveng.   With just 14 residents per home, this is not a facility, it's a family!

What is a Universal Worker?   

• Universal Workers provide hands-on care and support as a C N A, LPN, RN, SOCIAL WORKER
• Assist with daily household tasks like, cooking, laundry, and light housekeepiing
• Build meaningful relatiohships with residents in a close-knit setting
