Community Health Worker at Providence Santa Rosa Memorial Hospital in Santa Rosa, CA. This position is Full time and will work 8-hour, Day shifts.The Community Health Worker (CHW) is a trusted community member who acts as a liaison between a community and health/social service systems. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
