47-01 Queens Blvd., Suite B, Sunnyside, NY 11104 Schedule Candidates must be flexible to work rotating shifts, including: 6:45 AM – 3:15 PM 7:45 AM – 5:00 PM 11:30 AM – 8:00 PM Position Overview The Patient Care Coordinator supports the daily operations of a busy Optum healthcare environment by providing administrative, scheduling, and patient-facing support. This position works closely with a diverse team of approximately 60 professionals, including therapists, aides, authorization representatives, billers, and administrative staff. The ideal candidate is highly organized, professional, patient-focused, and comfortable working in a fast-paced medical office environment. Key Responsibilities Schedule, coordinate, and manage patient appointments Optimize provider and clinic schedules Check patients in and out of the clinic Collect and verify patient demographic information Accurately enter and maintain information within the electronic medical record Manage referrals and Plans of Care (POCs) Communicate with patients regarding insurance coverage Collect copays and outstanding patient balances Answer incoming telephone calls and make outgoing patient calls Address patient questions, concerns, and scheduling needs Maintain accurate and complete patient records Coordinate with clinical and administrative team members to support efficient patient care Provide professional, empathetic, and patient-centered customer service Complete additional assignments and projects as needed Required Qualifications High school diploma, GED, or equivalent 1–3+ years of healthcare, medical office, or related administrative experience Strong patient-facing and customer service experience Knowledge of healthcare administrative processes Strong scheduling and coordination skills Experience working with electronic medical records;

Epic experience is highly preferred Proficiency with Microsoft applications Strong verbal and written communication skills Excellent attention to detail Strong organizational and time-management skills Ability to multitask and prioritize responsibilities Strong problem-solving skills Ability to work efficiently in a fast-paced environment Flexible and adaptable to changing clinic needs Strong follow-through and dependability Professional, approachable, and empathetic demeanor Preferred Candidate Profile The ideal candidate will have previous experience in a healthcare or medical office environment and be comfortable interacting directly with patients throughout the day. Candidates should have a strong understanding of appointment scheduling, insurance-related patient communication, EMR documentation, and general front-office healthcare operations.
