RN Case Manager: Care Coordination & Discharge Planning

Baptist Health is seeking an RN with an Associate Degree to support discharge planning, utilization management and care coordination across pre-acute and post-acute settings. The role requires current RN licensure and strong communication and analytical skills. Candidates should have hospital clinical experience and ideally 1–3 years in case management.
