Salary Range: $40.55 - $59.30 per hour | Base rate + visit differential.

Sign-on Bonus: up to $5,000.00

How You'll Make An Impact In This Role

• Evaluate and assess home health patients with physical, emotional, cognitive, and psychosocial deficits, including their ability to safely perform independent living skills.
• Develop, implement, and modify an individualized plan of care, determining appropriate treatment type, frequency, modalities, and seeking consultation as needed.
• Monitor, document, and oversee care, tracking patient progress, supervising delegated services in compliance with regulations, and adjusting treatment accordingly.
• Educate and support patients and caregivers, assist with discharge planning, and provide guidance on adaptive equipment and community support resources.
• Independence, organizational, professional.

What Minimum Requirements You'll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.
• Rehab Therapist specializing in Occupational Therapy (OTR) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required.

Education:

• Bachelor's degree required. Master's degree preferred.

What Additional Preferences We're Seeking

• Bachelors of Science Occupational Therapy
• Experience in geriatric and home health strongly preferred.
