Key Responsibilities\n

• Conduct initial assessments and ongoing evaluations to identify clients’ needs and develop personalized treatment plans.

• Facilitate individual and family counseling sessions that adhere to ethical standards and best practices in social work.

• Collaborate effectively with a multidisciplinary team to coordinate care strategies and ensure comprehensive service delivery.

• Provide training and education to clients and families regarding mental health resources and coping strategies.

• Maintain accurate and timely documentation of client interactions and treatment progress in the designated electronic medical record (EMR) system.

• Stay informed of the latest developments in social work practices, especially in hospice care, to enhance service delivery.

Patient Population / Client Focus\n

This role primarily focuses on serving elderly populations receiving hospice care. Many clients may present with complex psychosocial support needs as they navigate end-of-life challenges. As a Licensed Clinical Social Worker, you will empower clients and their families by offering tailored support, assessing emotional responses, and facilitating access to additional community resources. Your insights will help shape care plans that reflect the unique preferences and cultural backgrounds of each client.

Qualifications\n

• A Bachelor’s degree in Social Work from an accredited institution.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Nevada.

• Proven experience in a role that required direct client interaction, preferably within a healthcare or hospice setting.

• Exceptional communication, interpersonal, and clinical assessment skills.

• Strong understanding of social work ethics and regulations as they pertain to hospice care.

Compensation and Benefits\n
