Job Description

The Case Management Registered Nurse will be responsible for providing comprehensive case management services to ensure optimal patient care. This role requires collaboration with interdisciplinary teams to facilitate and coordinate patient treatment plans.

Responsibilities

• Assessing patient needs and developing care plans.
• Coordinating with healthcare professionals to implement treatment strategies.
• Monitoring patient progress and outcomes.
• Providing education and support to patients and families.

Required Experience / Certifications / Licensure

• Active RN license.
• Previous experience in acute care case management.
• Familiarity with Meditech and Midas systems preferred.
