Case Manager Responsibilities Travel to provide in-home and community-based services to youth and families Manage a caseload of foster youth and support foster homes Coordinate services with schools, courts, medical providers, and community partners Conduct assessments and assist in developing individualized service and treatment plans Support family reunification and placement stability effortsMaintain timely and accurate documentation of visits, services, and case progress Build and maintain strong relationships with youth, families, and stakeholders Participate in team meetings, court involvement, and on-call rotation as needed Case Manager Qualifications Bachelor’s Degree in Social Work, Counseling, Psychology, Sociology, or related human services field LSW or LPC or license eligible Minimum 2 years of relevant experience preferred (child welfare or working with youth strongly preferred) Strong communication, organizational, and problem‑solving skills Ability to work independently and manage a flexible schedule Valid driver’s license, reliable transportation, and ability to travel daily Case Manager Benefits Strong base salary!
