Work Environment

This role is based onsite in a school setting, allowing you to operate within a vibrant community environment. You will be part of a collaborative care team consisting of physicians, nurse practitioners, physician assistants, medical assistants, and other healthcare support staff. Key Responsibilities

• Provide exceptional primary care by performing detailed evaluations, accurate diagnoses, and comprehensive treatment plans for patients.
• Conduct office visits for both routine check-ups and acute medical issues, delivering timely and responsive care.
• Administer annual wellness visits and health risk assessments, taking a holistic approach to patient health that includes thoughtful and specific documentation.
• Coordinate care with other healthcare providers, specialists, testing facilities, and agencies to ensure seamless patient management.
• Lead population health initiatives, ensuring compliance with preventive health measures such as breast cancer screenings for eligible patients every two years.
• Assist the care team with phone triage and outreach efforts to foster better communication and education regarding health conditions and care plans.
• Educate patients about their health conditions, treatment options, and care plans to empower them in their healthcare journey.
• Engage in promotional activities aimed at enhancing patient awareness and clinic visibility.
• Occasionally conduct home visits to provide care for patients who may have mobility challenges.
• Keep abreast of the latest healthcare practices and participate in team meetings while reporting directly to the Medical Director.

Patient Population / Client Focus

The primary focus of this role is on older adults enrolled in Medicare. These patients often face chronic health issues and require a compassionate, personalized approach to care that emphasizes prevention, education, and active management of their health. By working closely with this patient demographic, you will have the opportunity to foster long-term relationships, build trust, and significantly impact their quality of life.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or graduation from an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license, along with an active DEA license.
• Demonstrated experience in primary care settings, particularly within internal medicine, geriatrics, or family medicine.
• A strong understanding of HCC documentation, ICD-10 coding, and health risk assessments.
• A collaborative spirit, eager to work as part of a dynamic healthcare team.
• A desire to be involved in an innovative care model focused on empirical population health.
• Bilingual proficiency is highly valued to assist diverse patient populations.

Preferred Experience

• Experience within a school-based health model or community health setting would be beneficial.
• Previous involvement with value-based care initiatives or population health management is a plus.

Compensation and Benefits

The salary for this role ranges from USD 180,000 to 200,000 annually, depending on experience and expertise. Additionally, we offer a competitive benefits package that includes:
