Work Environment

This position is based onsite in a school setting, providing an opportunity to work closely with students and families, enhancing their health knowledge and access to necessary services. You will be part of a vibrant and collaborative health team, sharing insights and strategies to improve health initiatives. Your work schedule typically runs from 9:00 a.m. to 6:00 p.m., with occasional evening clinics that extend until 8:00 p.m. Additionally, you may participate in community outreach events on weekends, allowing you to engage directly with the community.

Key Responsibilities

• Conduct thorough health assessments and screenings to evaluate the needs of individuals and families, utilizing tools such as electronic health records for accurate documentation.
• Diagnose and manage both acute and chronic medical conditions, implementing evidence-based practices to deliver preventive care and health promotion tailored to each patient.
• Collaborate with local organizations, schools, and stakeholders to design and execute impactful health education initiatives that address community-specific health issues.
• Provide culturally competent care, advocating for patients' rights, and ensuring fair access to healthcare services for all community members.
• Coordinate referrals and manage follow-up care to promote continuity and quality of care, utilizing healthcare networks and resources effectively.
• Maintain meticulous medical records in accordance with regulatory requirements, ensuring confidentiality and compliance standards are met.

Qualifications

• Possession of a current Nurse Practitioner certification.
• Valid and active nursing license in the state of New York.
• A Bachelor's degree in nursing from an accredited institution; preference for candidates with a Master's degree.
• A minimum of 3 years of clinical experience in a healthcare setting, ideally in community health or public health environments.

Compensation and Benefits

• A competitive salary package ranging from USD 180,000 to USD 200,000 annually, commensurate with experience and qualifications.
