Licensed Practical Nurse (LPN) – Post Acute Job Type: Full-Time | Nights Schedule: 3x12; 7p‑7a; Rotating Weekend Location: Highpoint Health Winchester with Ascension Saint Thomas, Tennessee. Key Responsibilities Provide direct patient care under RN supervision, following scope and standards of practice. Collect data for assessment and collaborate with RN to contribute to patient evaluation. Perform plan of care interventions including specimen collection, wound care and medication administration. Document patient care and assist RN in developing the plan of care. Assist patients with activities of daily living: personal hygiene, elimination, nutrition, ambulation. Monitor, document, and communicate patient status and reactions to interventions. Advocate for physical and emotional well‑being of patients. Qualifications Current Tennessee or compact state LPN license and graduate of a practical nursing program. Valid AHA Basic Life Support (BLS) certification within 30 days of hire. Critical thinking, decisive judgment, ability to work independently in a stressful environment. One year of previous experience preferred.
