Key Responsibilities\n

• Conduct thorough assessments and evaluations of patients and their families to identify social, emotional, and economic factors impacting their health and treatment outcomes.

• Develop and implement individualized treatment plans that address the unique needs of each client, utilizing evidence-based practices and trauma-informed care approaches.

• Research, refer, and advocate for community resources, including food assistance programs, childcare services, and healthcare options, to enhance client well-being.

• Respond promptly to crisis situations, including instances of child abuse or mental health emergencies, providing immediate support and intervention.

• Monitor ongoing client situations through regular follow-up interactions, ensuring continuous improvement and adjustment of treatment plans as necessary.

• Identify and engage with populations and communities in need, working to break down barriers to access services.

Patient Population / Client Focus\n

This role primarily focuses on clients experiencing substance use disorders and the associated trauma. You will work closely with a diverse population, including individuals from various socio-economic backgrounds. Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Valid California LCSW License.

Compensation and Benefits\n
